What Needs Attention

  • Task completion lag pattern: The system struggled to detect when work was already done. A frontmatter fix task sat in “Now” for 20+ hours before someone checked and found the files were already correct. Consider automated escalation or state verification for tasks stalled >12 hours.

One Insight

When autonomous systems manage their own task queues, “stuck” doesn’t always mean “blocked” — sometimes it means “done but not recognized.” Building in explicit completion verification could prevent phantom bottlenecks.
